Niacinamide

A well-tolerated vitamin B3 derivative with controlled human evidence for modest improvements in visible photoaging, pigmentation, and skin-barrier function.

What the evidence says

A 12-week double-blind split-face trial in 50 women found that 5% niacinamide improved fine lines, hyperpigmented spots, texture, red blotchiness, sallowness, and elasticity versus vehicle. An eight-week trial in 30 women also reported improvement in periorbital wrinkles and skin-surface roughness with 4% niacinamide.[1][2]

Controlled pigmentation studies found that topical niacinamide reduced facial hyperpigmentation and melanosome transfer. In a small melasma trial, 4% niacinamide produced good-to-excellent improvement in 44% of participants versus 55% with 4% hydroquinone, with fewer and milder adverse effects; both groups also used sunscreen.[3][5]

The evidence is not limited to appearance. A human study found that topical nicotinamide increased stratum-corneum ceramides and free fatty acids and reduced transepidermal water loss, supporting a measurable barrier effect.[4]

Confidence remains moderate rather than strong. Trials are short, mostly involve women, and several were conducted or supported by manufacturers. The useful evidence centers on well-formulated 2% to 5% products; it does not show that fashionable 10% to 20% serums work better.[6][1]

These findings concern visible photoaging, uneven pigmentation, and barrier function. They do not establish prevention of skin cancer, reversal of intrinsic aging, or any whole-body longevity effect.[6][3]

Potential benefits

  • May modestly improve fine lines, uneven pigmentation, texture, and redness over several weeks.[1]
  • Can improve epidermal barrier lipids and reduce water loss from dry skin.[4]

Side effects and cautions

  • Local stinging, dryness, redness, or irritation can occur, although controlled trials generally report good tolerability.[6][2]
  • Evidence from one concentration or vehicle cannot be assumed to apply to every high-strength commercial serum.[6]
