What the evidence says

Adapalene is a topical retinoid developed for acne. Systematic and network reviews support it as an effective treatment for inflammatory and noninflammatory lesions, often used alone or with benzoyl peroxide depending on acne severity.[1][2]

The Strong rating applies to acne. FDA labeling for 0.3% gel covers acne in people aged 12 years and older, with a thin film applied to the affected area rather than spot treatment. Dryness, redness, scaling, and burning are most common in the first month.[3]

Photoaging use is off-label and less certain. A 24-week randomized comparison found 0.3% adapalene noninferior to 0.05% tretinoin for several clinical photoaging measures, and a newer placebo-controlled trial of 0.1% cream reported improvements in wrinkles and pigmentation but also frequent stinging and xerosis.[4][5]

Adapalene is not a quick cosmetic resurfacer: acne can appear worse early, benefits take weeks, and irritation increases when combined too aggressively with peels, scrubs, waxing, or other retinoids. Sun protection remains important, and pregnancy or pregnancy planning should be discussed with a clinician despite low topical absorption.[3][1]

  • May improve visible photoaging, although this use has a much smaller evidence base and is off-label.[4][5]

Side effects and cautions

  • Commonly causes dryness, peeling, redness, burning, stinging, or itching during early treatment.[3]
  • Irritation can worsen with abrasive products, waxing, other retinoids, or excessive sun exposure.[3]
