Berberine

What the evidence says

Berberine is an alkaloid found in barberry, goldenseal, and related plants. Reviews in type 2 diabetes report lower fasting glucose and HbA1c, including a 2022 meta-analysis estimating an average HbA1c reduction near 0.6 percentage points, but most studies were short and many were conducted in China.[1][2]

A placebo-controlled lipid meta-analysis of 18 studies found modest reductions in LDL cholesterol and triglycerides over 4 to 24 weeks. These biomarker changes do not show that berberine prevents heart attack, stroke, kidney disease, or other diabetes complications, and it should not be presented as a natural equivalent to any single prescription drug.[3]

Product and dose differences matter because supplements may contain berberine from different plants or extracts. NCCIH describes possible adjunctive benefit but highlights variable effects and poor-quality studies; substituting it for proven diabetes or lipid treatment can delay effective care.[4][1]

Nausea, diarrhea, bloating, and constipation are common. Berberine can affect drug transporters and metabolizing enzymes, creating interaction risk across many medicines. It should be avoided during pregnancy and breastfeeding and not given to infants because bilirubin displacement can contribute to severe neurologic injury.[4][5]

Side effects and cautions

  • Commonly causes nausea, diarrhea, constipation, bloating, or abdominal discomfort.[4]
