What the evidence says

Plant sterols and stanols resemble cholesterol and reduce its intestinal absorption. They are commonly added to spreads, dairy-style drinks, and other foods; the evidence concerns a regular daily dose from these products, not an occasional capsule.[1]

A meta-analysis of 84 controlled trials found a continuous dose-response relationship, with roughly 2 grams per day lowering LDL cholesterol by about 9%. Taking more produced diminishing additional reductions, so these products are an adjunct rather than a substitute for a broader lipid-lowering plan.[1][2]

Sterols and stanols both work, and aggregate comparisons do not establish a clinically important difference at usual doses. Food format, timing with meals, background diet, and adherence help explain why results vary between products and people.[2][3]

The European Food Safety Authority accepts that 1.5 to 3 grams daily can lower LDL cholesterol, while also emphasizing that the claimed effect is on a risk factor. No randomized trial has shown that plant-sterol products themselves prevent myocardial infarction, stroke, or death.[4]

An updated umbrella review continues to find meaningful LDL lowering but does not turn biomarker evidence into direct clinical-outcome evidence. People with the rare inherited disorder sitosterolemia should avoid supplemental plant sterols, and anyone already using lipid-lowering medication should treat them as part of the same clinical plan.[5][4]

Side effects and cautions
