What the evidence says

WHO recommends muscle-strengthening activity involving all major muscle groups on at least two days per week. Free weights, machines, resistance bands, and appropriately scaled bodyweight movements can all provide progressive resistance.[1]

A Cochrane review of 121 randomized trials found that progressive resistance training produces a large improvement in strength and smaller but meaningful improvements in physical function, gait speed, and chair-rise performance in older adults.[2]

Very old and frail adults retain substantial adaptive capacity. A landmark nursing-home trial reported strength gains exceeding 100% after ten weeks of supervised high-intensity progressive training. Its small size does not establish long-term durability or a mortality effect.[3]

Prospective-cohort meta-analyses associate resistance training with roughly 10% to 17% lower all-cause mortality and suggest the lowest observed risk around 30 to 60 minutes per week. These self-reported observational data cannot prove that lifting itself caused longer life or that higher volume is harmful.[4][5]

Training should progress gradually with attention to technique, balance, recovery, and existing orthopedic or cardiovascular disease. Temporary soreness is common, while frail adults or people with unstable conditions may benefit from supervised modification.[6][1]
